Observation

(1) Staff working in the Discovery Preschool classroom reported it was challenging to get through the diapering process with all the children, especially when working alone. On multiple dates in October and November 2022 there were periods of 3 to 4 hours where there was no documentation of children's diapers being checked or changed by staff. Further, when one child was moved to another classroom without diapering facilities, this resulted in withholding hygiene care or diaper changing from a child unable to do that for themselves because the staff in that room do not change diapers. It was determined that this allegation is ‘valid’ for withholding hygiene care or diaper changing. WAC 110-300-0331(1)(g)(viii)
